Application letter for name change

Dear sir,

Sub : Discrepancy in spelling-reg.

Ref : My second year mark-sheet

I wish to bring to your kind notice that my name is spelt wrongly in the mark-sheet. My original spelling is 'Sreedevi'; but it is spelt as 'Sridevi'. I have enclosed my 12th std mark sheet for your perusal and necessary corrections.
